AVOLA Therapy partners with physicians across the Phoenix West Valley to provide in-home occupational and physical therapy for your patients. We make the referral process simple and keep you informed throughout the course of care.

We often schedule the initial evaluation in just a few days of receiving a referral, depending on therapist availability, patient location, and any intake or benefits requirements. Patients begin treatment as quickly as practical, which helps support continuity of care.

Therapy in the patient's home allows our clinicians to assess the actual living environment, identify fall risks, recommend modifications, and train patients on functional tasks that matter in their daily lives.

AVOLA Therapy is a qualified vendor with the Arizona Division of Developmental Disabilities (DDD). We meet the state's requirements to deliver therapy services to DDD members.

Medicare-Enrolled

We are a Medicare-enrolled provider meeting all federal standards for therapy services under the Medicare program.

You receive thorough evaluation reports, progress notes, and discharge summaries. Our documentation is timely, compliant, and designed to give you a clear picture of your patient's status.

Referral Process

1. Send a referral with the patient's name, date of birth, diagnosis (ICD-10 code), and relevant medical history
2. We contact the patient as promptly as possible, often within one business day, to begin scheduling the initial evaluation
3. Evaluation and plan of care are completed and sent to you for review
4. Ongoing updates are provided throughout the course of treatment

Direct Access Note

Under Arizona's Direct Access law, many patients can begin occupational and physical therapy without a physician referral. However, we welcome and encourage physician referrals to ensure coordinated care. If a patient's insurance plan requires a referral for coverage, we communicate that during the benefits verification process.

Licensed Clinicians

All therapy is provided by licensed Occupational Therapists (OT), Occupational Therapy Assistants (OTA), Physical Therapists (PT), and Physical Therapy Assistants (PTA) with current Arizona licensure.
