How Insurance Works with AVOLA

AVOLA Therapy works with most major insurance carriers on both an in-network and out-of-network basis. This means:

• You choose your therapist -- you are not limited by a narrow provider network
• We submit claims on your behalf to your insurance company
• Your costs depend on your plan -- in-network plans typically have lower out-of-pocket costs, while out-of-network plans may reimburse a portion or all of your therapy costs

Verification Process

Before your first visit, we make it easy to understand your coverage:

1. Provide your insurance information. Share your insurance card and we do the rest.
2. We verify your benefits. Our team contacts your insurance company to determine your therapy benefits, whether in-network or out-of-network.
3. We explain your costs. Before treatment begins, we walk you through what your plan covers and any estimated out-of-pocket costs.

What to Expect: Copays and Deductibles

Every insurance plan is different. Here are common terms to understand:

• Deductible: The amount you pay before your insurance begins covering services. Some plans have separate in-network and out-of-network deductibles.
• Copay / Coinsurance: Your share of the cost for each visit after your deductible is met. Out-of-network coinsurance rates vary by plan.
• Out-of-pocket maximum: The most you will pay in a plan year. Once reached, your insurance covers the remaining costs.
